Continuing Care Programme

Tabor Group’s Continuing Care Programme is a year-long, extendable programme designed to provide ongoing support and guidance to people transitioning from residential treatment to living independently or going home. Recovery is a lifelong journey, and the early stages, particularly the first year after treatment, can be challenging as people face the realities of daily life without the structure of a residential programme.

Our Continuing Care Programme is a key component of maintaining long-term recovery, offering a vital safety net as clients rebuild their lives. The programme features weekly, peer-led facilitated group meetings where people can share their experiences, seek advice, and learn coping strategies to handle everyday challenges. These sessions offer a supportive environment, enabling people to build connections with others who understand the complexities of addiction recovery.

In addition to peer support, each person follows an individualised Care Plan tailored to their unique needs and goals. This Plan evolves over the course of the year to adapt to the person’s progress and any emerging challenges.

We recognise that recovery is not a linear process, which is why we offer the flexibility for people to extend their participation in the programme for an additional year, if needed. This extended support ensures that individuals continue to receive professional guidance and peer encouragement as they further strengthen their recovery skills and resilience.

Our ultimate goal is to empower people to confidently navigate life’s ups and downs while maintaining their sobriety. The Continuing Care Programme supports recovery from addiction and overall well-being, helping people to thrive personally and professionally.
